Duties and Responsibilities:
Objective 1: Identification of Cases
- Identifies individual cases in collaboration with the team and through regular presence in the community;
- Accepts referral from other agencies and community partners;
- Work closely with the other organization in order to ensure smooth referral.
Objective 2: Case Management
- Provides individual case management for beneficiaries and their families, in accordance with all case management steps and related tools and procedures;
- Ensures that all cases are addressed immediately and shared with the supervisor;
- Provides ongoing support to beneficiaries
- Directly communicates with other caseworkers and service providers as part of conducting individual case management;
- Regularly attend case management meetings.
Objective 3: Consistently and proactively monitors/assesses the safety and security of field teams; promptly reporting concerns or incidents to management and liaising with community leaders and other external parties as required to maintain/enhance the security environment for programs.
Objective 4: Reports, Capitalization and Communication
- Ensure the correct documentation and compilation of CM forms in accordance with all case management steps and related tools and procedures, and the National SOP;
- Updates regularly the database to ensure a comprehensive record of the database;
- Communicates regularly with supervisor, for updates on achievements and challenges;
And any other duties seemed relevant to the position and assigned by your manager.
