Description: 

Under the direct supervision of the PHC Quality Officer, the PHC Social Worker will be responsible for supporting PHC staff and meeting the unique needs of each beneficiary in the targeted areas. He /She will oversee the activities conducted by the community outreach workers to ensure proper outreach at community level.

Responsibilities and Essential Functions

  • Guide patients and provide them with the needed information on existing services at the supported PHC
  • Establish a good communication network with the different actors in intervention to facilitate the implementation of project activities
  • Identify vulnerable individuals and provide them with the best opportunity to connect to care services
  • Conduct a full assessment of beneficiaries’ needs and collaborate with other professionals to evaluate patients' medical condition and to assess their needs
  • Provide needed assistance to the targeted beneficiaries and make necessary and timely referrals to specialized health care services whenever needed, respecting confidentiality and the beneficiary’s right.
  • Refer beneficiaries to other services as needed
  • Follow up with patients to ensure their access to the right services
  • Follow up and ensure proper implementation of the LPSP at the supported PHCC.
  • Provide awareness sessions at the PHC level, and community if needed, on several topics, such as, but not limited to, vaccination, non-communicable diseases, IYCF, nutrition, ECD…
  • Support the PHCC with data entry and ensure proper and timely reporting
  • Monitor the implementation of the project activities at the supported PHCC and report problems, challenges and issues to the PHC Quality Officer
  • Hold a monthly meeting with the PHC Quality Officer to follow up on the status of the activities being implemented at PHCC level
  • Supervise, support and coach the Community Outreach Workers and ensure proper outreach activities of optimum quality.
  • Conduct field visits to monitor the work of the Community Outreach Workers, identify gaps, guide and support them to reach their targets.
  • Coordinate with the focal person of the MoPH PHC department to ensure proper activities following the MoPH guidance and procedures.
  • Ensure that the supported PHCCs adhere to MoPH reporting standards and deliver the packages of support as per the agreement between PHCCs and IOCC.
  • Prepare the monthly payment files of the PHCC in charge and submit them timely to IOCC’s PHC Quality Officer.
  • Work closely with the Medical Specialist and follow up on the guidance and approvals on diagnostic medical tests/acts and ensure proper documentations on the patient files and claim forms, following IOCC’s procedures.
  • Prepare for the Medical Specialist site visits and support with the needed tasks.
  • Ensure timely reporting of the activities to the PHC Quality Officer
  • Perform other duties as required or assigned

Qualifications

Knowledge, skills and abilities:

  • Good organizational skills.
  • Excellent communication and interpersonal skills.
  • Self-motivated and initiative taker. 
  • Good report writing skills.
  • Ability to communicate with vulnerable populations.
  • Dynamic personality.
  • Proficiency in Microsoft Office.
  • Fluency in written and spoken Arabic and English; French is a plus.

Education:

  • B.Sc. in social work or any other relevant field.

  

Experience:

  • 2-3 years of work experience.
  • Previous experience in NGO and humanitarian field is an advantage.
  • Previous experience in working with children and vulnerable populations is an advantage.

                                                                                                                         

Working Conditions

  • Stationed in the supported PHCs in Nabatiyeh and Marjeyoun areas, with frequent visits to IOCC office, and field visits to the catchment area of the supported PHCC as needed.
