Contracting Biomedical engineer

Scope of Work

The purpose of this assignment is to seek a contracting Biomedical engineer, this includes the provision of Quarterly Preventive Maintenance or PM (the regular and routine maintenance of medical equipment in order to keep them running and prevent any costly unplanned downtime from unexpected equipment failure) and on-call corrective maintenance or CM (Task that corrects a problem with a medical equipment and returns it to proper working order) listed in “Para. 12 – Commercial Offer” Form for primary healthcare center (PHC) and dispensary equipment located in 2 Amel Association PHC Centers (Sour and Bazourieh) for the period 1 year subject to further extension of similar period based of satisfactory performance and with the consent of Amel Association senior management.

Amel Association wishes to receive proposals from firms with the ability to provide high quality PM / CM services as per below Terms of Reference (ToR) – Para. 6 3. RFQ submission

Request for clarification:

Bidders are required to submit any request for clarification in respect of this RFQ by email: [email protected].

The deadline for receipt of question is 30/06/2022 The questions from all the suppliers would be consolidated by the Procurement Department and answered collectively. To ensure that the same level of information is conveyed equally to all participating suppliers, response to a question raised by one supplier can be shared with all the other suppliers. Terms of reference (TOR)

Licensing: All Biomedical Engineers should be licensed by the Ministry of Public Health and medical occupations department in the ministry, and be member of Order of Engineers and Architects in Lebanon.

Service Coverage: The Biomedical Engineer should be able to cover all the following tasks:

1. Having a first look at the center's equipment.

2. Listing all the equipment in a list similar to the attached excel sheet, for each center.

3. Labeling of each piece of equipment with the following: 1. Name of equipment 2. Code 3. Date of PM 4. Date of upcoming PM 5. Visit report (includes what was done during every visit to the center). 4. Conducting Quarterly Preventive Maintenance in the 7 PHCCs. 5. Providing their contact number and responding to center's calls for CM as agreed within the contract.

- Reporting: Service provider should provide Amel Association with the visit reports and/or maintenance logs within 7 days from the visit date.

- Alarming results: In case of alarming results the service provider has to inform Amel Association through the phone without waiting till report is provided.

- Urgent cases: Upon request from Amel Association, the service provider has to prioritize some visits based on the case urgency.

- Sample integrity: It is the responsibility of the service provider to ensure the integrity and proper handling of the PHC equipment.

- Responsibility: In case of contradiction between the obtained report and the actual performance of equipment, the service provider has to repeat the test to confirm the visit without any extra expenses.

Period of validity

Your quotations should be valid and binding for at least for 30 days from the date of submission of the RFQ. PR#00929 8.

Documents required

- Biomedical Engineer ID Card

- Certificates of Education.

- The order of Engineers and Architects card.

- New Criminal Record - 3 months old.

- Acknowledgement of enclosed “Statement of Integrity”

- If your firm is not registered with Amel Association, you should complete, sign and submit the Vendor Pre-Qualification Form “Annex A”. -

Please find attached in “Annex B” Amel Association Suppliers Pact of Conduct.

Offers that doesn’t contain your acknowledgment of Amel Association Suppliers Pact of Conduct will result in disqualification of your offer.

Subcontracting Under no circumstances is the service provider allowed to sub-contract any of the services quoted above without informing Amel Association. Failure to abide by this results in immediate contract termination.

Confidentiality All information shared by Amel Association including staff and patient information and equipment position are confidential and shall not be shared with any external parties. Failure to respect Amel Association privacy would result in immediate contract termination.

Terms of payment Upon the receipt of an invoice, Amel Association would within 10 days pay the requested amounts after the validation by the project coordinator and finance officer.
