You are invited to quote on supplying MAP with the products specified below .

1. Requested products: Medications-Injectables

Injectable            Ipratropium Br (Atrovent) 500 mg            (2000 Amp)    

Injectable            Metoclopramide HCl 10mg/2ml IV/IM    (4000 Amp)      

Injectable            Omeprazole 40 mg/10 ml IV        (2000 Amp)      

Injectable            Ondansetron 8mg            (1500 Amp)      

Injectable            Tetaglubin (Human Tetanus Immunoglobulin) 250 IU          (200 Vial)     

* Final quantities will be determined based on prices **Please add VAT only if the item is subject to VAT. If any of the requested items is not available, please provide a quote for the items you can provide.

1. Offers Submission: all the offers must be sent via e-mail only to [email protected] any offer emails sent to other emails or additional emails will be rejected.

         The offer should include:

         Annex A. Registration Certificate of the company with MOF Certificate and owner’s ID or passport.

         Annex B. Financial Proposal signed, and stamped with complete information (You can fill the above table)

1. Deadline for Submitting Offers: your offer must be submitted by Wednesday October 5, 2023 at noon.
2. Payments: by bank transfer from abroad (fresh money) within one week following receiving of the invoices, delivery notes and acceptance of delivered items.
3. Bank charges: MAP will not be responsible for any bank charges whether local or at intermediate bank. Suppliers must take this issue into consideration when submitting offers.
4. MAP is not responsible for any variation in prices due inflation, customs, fluctuation in exchange rates or any other market factors after receiving the quotation.
5. No additional costs related to transporting, loading, and unloading the items should be charged to MAP; Bidders must take this issue in consideration when submitting offers.
6. Order nature: this offer is requested in an emergency; the final items quantities will be adjusted according to the prices.
7. Manufacturer and country of origin: Please specify the brand name and the country of origin. Offers that doesn’t specify brands and country of origin may not be considered.
8.  The products have a long shelf life (not less than one years, when it is applicable) and meet international standards.
9. Availability: the offered quantities should be available upon request within 2 weeks without any shortages, if full quantity is not available, please specify the available quantity. In addition, if quantities need more than the requested period, please identify the needed period for delivery.
10. Offer validity: the offer should be valid for one month.
11. Credentials: For your offer to be considered, you should provide it with the legal documents (certificate of registration at the ministry of finance, passports or IDs of the shareholders whom shares are over 20%).
12. Decision Criteria: best price for quality. Multiple suppliers might be contracted. For technical evaluation, shortlisted candidates might be asked to provide samples.
13. Delivery Timeline: within maximum 2 weeks of signing the purchase contract (penalty might apply when delivery time is missed).
14.  Delivery Location: To Palestine Red Crescent Society (PRCS) hospitals warehouse in Saida – next to Hamshari Hospital.
15. Logo stickers will be provided for suppliers to be placed on boxes
16. MAP will not be responsible for any damage or loss to the products before signing the relevant delivery note.
17. At delivery time checking on all delivered equipment will be made by MAP. MAP holds the right to reject any products not meeting the quality standards, not in line with the offer provided and purchase order.
