Key responsibilities
Accountability and Complexity
Under the supervision and guidance of the public health officer, RI nurses will monitor the referral process of pregnant women and ensure they receive adequate medical care, based on their professional judgment in hospital settings.
Complex care coordination: RI nurses will coordinate between hospitals and primary health care centers, managing multiple aspects of care from hospital admission to post-natal visits.
Effective communication: RI nurses must possess excellent communication skills to effectively liaise between patients, healthcare providers, PHCCs and hospitals.
Patient care in Hospitals:
Receive referrals of High-Risk pregnancies: Assess referrals of pregnant women who are at high risk due to conditions such as preeclampsia, gestational diabetes, or multiple gestations and coordinate with RI’s field medical coordinator for approval on admission.
Coordinate Referrals: Facilitate referrals to hospitals for further evaluation and management. Ensure that referrals are managed in a timely manner and that the pregnant women are aware of their treatment plan.
Monitor and Follow-Up: Continuously track the progress of high-risk pregnancies management through regular follow-ups. Ensure that the women attend their specialist appointment, are admitted promptly, adhere to treatment plans, and receive the necessary care to manage their risk effectively.
Facilitate the Admission and Treatment of children under 18: Following the identification of patients and referrals from the supported PHCC, RI nurse will coordinate with hospitals to ensure that children are admitted promptly and receive the required interventions.
Ensure Comprehensive Documentation: Following RI guidelines, the nurse will have to ensure full documentation by hospital staff for all patient interactions, including medical histories, care provided, and follow-up visits. This includes maintaining accurate copies of medical files and referral documents to support continuity of care and facilitate effective coordination with health facilities.
Coordination with PHCCs:
Schedule Appointments: Collaborate with Primary Health Care Centers (PHCCs) and RI’s nutrition team to schedule post-natal care visits for new mothers. Ensure that these appointments are arranged promptly to support continued medical assessment and care.
Ensure Continuity of Care: Communicate effectively with PHCCs to ensure that the new mothers are receiving the necessary follow-up care.
Provide Education and Resources: identify needs for capacity building and coaching for hospital staff, and coordinate with the PHO to develop and implement a training plan accordingly.
