Health Outreach Consultancy

Responsibilities

• Map, identify and screen vulnerable beneficiaries in need of nutrition support
• Conduct outreach in all pre-identified location
• Upload data on daily basis
• Support in preparation of outreach work and conduct frequent outreach visits with identified communities
• Counseling and case management for beneficiaries (pregnant women, lactating women and children) in need of nutrition care, ensure proper referral and follow up
• Conduct outreach follow up based on needs
• Conduct awareness sessions to beneficiaries on nutrition
• Support the identification and coordination of outreach events and activities related to health and nutrition
• Support the referral of pregnant women, lactating women and children
• Report on challenges encountered, good practices and recommendation for the program
• Responsible for the implementation of required outreach interventions and deliverables based on the aim of the project
• Assist with planning work on weekly and monthly basis in accordance with the Outreach work plan
• Accurately collect data during outreach activities and maintain confidentiality concerning target beneficiaries
• Report on daily basis on the outreach progress and on challenges faced to the supervisor
• Develop an understanding of the community's culture and respond to its challenges
• Weekly report for the health status of the beneficiaries served
• Documentation and analysis data required
• Report to the supervisor on weekly basis all health activities

Qualifications

Requirement

• Preferable experience in the community health field
• Proven experience for 2 years working in NGOs

Knowledge and Skills

• Good skills and experience working in outreach and community awareness raising
• Must have a good knowledge of and familiarity with the target areas, including local community and population habits and behaviors, focal points or local leaders within the community, concentration of vulnerable population in specific areas.
• Must be skilled in the use of Excel and Word.
• Should be able to communicate effectively, both orally and in writing, clearly articulate information, be a good listener and able to establish positive relationships and build trust with people at all levels
• Should be able to develop and respect work plans, work independently, manage time effectively, and meet deadlines

Computer skills

• Excellent knowledge in MS office
