Ojective 1: Identification of Cases
Identifies individual cases in collaboration with the CP team and through a regular presence in the community;
Accepts referrals from other agencies and community partners;
Work closely with the other organization in order to ensure smooth referral.
Objective 2: Case Management
Provides individual case management for children and their families, in accordance with all case management steps and related tools and procedures;
Ensures that all cases are addressed immediately and shared with the supervisor;
Provides ongoing support to families, foster and alternative care systems;
Directly communicates with other caseworkers and service providers as part of conducting individual case management;
Regularly attend case management meetings at field level.
Objective 3: Participate actively in weekly case management team meetings, and collaborates closely with the other members of the CM team and the CP team.
Objective 4: Consistently and proactively monitors/assesses the safety and security of field teams; promptly reporting concerns or incidents to management and liaising with community leaders and other external parties as required to maintain/enhance the security environment for programs.
Objective 5: Maintains good relationships with all the local and Governmental actors present in the area.
Objective 6: Reports, Capitalization and Communication
Ensure the correct documentation and compilation of CM forms in accordance with all case management steps and related tools and procedures, and the National SOP;
Updates regularly the database to ensure a comprehensive record of the database;
Communicates regularly with supervisor, CM Project Manager and CM Specialist for updates on achievements and challenges;
