Background:

S/he is a member of the field area team which includes the field medical advisor, the area pharmacist, the senior health officer, the health officers, the quality officers, and the program assistants per area.
Health program provides both technical and financial support to local primary health care centers to enable provision of affordable, accessible, and quality healthcare services to refugee and vulnerable host population.

Job Purpose:

He/she is responsible for coordination of care pathways following a case management Model for people with disability (PwDs) who have complex care needs.

Main Responsibilities:

In accordance with established project guidelines, the Inclusion Officer will be the focal point in ensuring that target population receives intended services in the assigned area. S/he will:
1. Multidisciplinary Case Management: Serve as the main focal point (case manager) for coordination, monitoring, and reporting referrals and service provision within his/her area for the case management.
- Will implement and monitor the case management plan of identified cases according to set criteria, for moving PwD through the process of change, and toward the goal of self-sufficiency and achieving their highest level of functioning
2. Provision of Assistive Devices: Acts as focal point, in coordination with IMC medical advisors, IMC Senior Inclusion Officer, and PHC Staff for approval, coordination, monitoring, and reporting of provision of assistive devices at PHCCs or externally contracted Rehabilitation Centers
3. Provision of Rehabilitation Therapy: Acts as focal point, in coordination with IMC medical advisors, IMC Senior Inclusion Officer, and PHC Staff for approval, coordination, monitoring, and reporting of provision rehabilitation therapy (physiotherapy, occupational, speech therapy) at PHCCs or externally contracted Rehabilitation Centers
4. Conduct monitoring visits to PHCCs and contracted referral centers
5. Monitor service delivery quality in contracted referral centers (diagnostics/rehabilitation centers) and reports challenges.
6. Follow up on data entry inside PHCCs and monitor quality and accuracy of data entered and provide support for data entry clerks.
7. Monitor and report any challenges and/or gaps identified to inform adjustments to the program
8. Complete project documentation for assigned activities. Assist with identifying information for case studies and reports on promising practices.
9. Participate in meetings with area manager and community health workers to Track indicators and prepare weekly reports on PWD project.
Perform any other duties as requested by Health Area Manager

Essential Qualification:

  • Bachelor’s degree in social work – Physical therapy – Occupational Therapy -Bachelor’s degree in health-related field (public health/RN) with experience working with PWD and case management Or equivalent

Desirable Qualification:

  • Experience working with PwDs, inclusion and case manangement

Essential Experience:

  • Minimum of 2 years of work experience in project support, preferably related to disability

Desirable Experience:

  • NGO experience Experience monitoring projects and collecting relevant data preferred

Essential Competency:

  • Good command of written and spoken Arabic and English
  • Observation, active listening and analysis skills with ability to make sound judgment
  • Good relationship management skills and the ability to work closely with local partners and community members
  • Attention to detail, accuracy and timeliness in executing assigned responsibilities
  • Proactive, resultsoriented and service-oriented

Desirable Competency:

  • Able to take initiative within a defined framework
  • Microsoft office skills
