1 – Overview:

Amel Association International (Amel) is a Lebanese and non-sectarian NGO created in 1979. Amel works through 30 centers, 6 mobile medical units, 2 mobile education units and 1 protection unit, and for more than 40 years has supported the poorest regions of Lebanon, from Beirut and its South Suburbs to Mount Lebanon, Bekaa and South Lebanon. Amel offers accessible services for all in the following fields: food security, health, education, protection, child protection, gender-based violence, livelihood, promotion and protection of human rights, rural development

2 – Scope of work:

The purpose of this assignment is to seek a contracting diagnostic services supplier, this includes the provision of laboratory tests and imaging listed in “Annex A” – Commercial Offer Form located in the catchment areas of the Amel PHCCs in Ersal where Amel will be implemented for the period 1 year subject to further extension of similar period based of satisfactory performance and with the consent of Amel senior management Amel wishes to receive proposals from firms with the ability to provide high quality Diagnostic and imaging services as per the Terms of Reference (ToR) – Para# 6 within the RFQ

3 – The quotation must include:

Same requested specification

Prices in USD

4 – Required Documents:

All Bidders are required to send the below requirements procurementdpt@amel.org with the subject “RFQ - 04231 - Lab & Imaging services - Ersal” copying procurement@amel.org and procurementfield@amel.org  :

A signed and stamped quotation using “Annex A - LIDT - Commercial Offer”

Certificate of the registration in the ministry of finance (الشهاد المالية). and all related legal papers including owner ID.

Scanned copy of the laboratory or radiology center registration documents

Scanned copy of the practice license for the responsible physician (Pathologist/Radiologist) as applicable

If your firm is not register with Amel, you should complete, sign and submit the Vendor Pre-Qualification Form “Annex B”

Acknowledgment for the attached “Annex C - Amel Suppliers Pact of Conduct”

5 – Payment Terms:

Bank transfer.

7 – Timeframe

Quotations and the requested papers should be submitted on or before Monday 15-01-2025

8 – Important

Kindly note that the quotation will be automatically rejected if

The bidder doesn’t send all the requested papers (mentioned in number 4).

The quotation doesn’t include the requirements (mentioned in number 2).

The quotation is received after the deadline.
